If, as seems increasingly likely, we do lose the three points, as Chief Operations Manager, the ultimate blame for this error has to go to Paul Douglas, & he MUST fall on his sword
I wouldn't want to name names at this stage but certainly if it transpires that someone has genuinely effed up - they need a degree of accountability. I'm sure Tone would be in no mood to let it slide either

Would you really want someone to lose their job, income and all that this entails for loved ones (including children perhaps) because of an administration error?
Most of the time errors are institutional. If a mistake has been made its better to investigate and put in place all the learning from it.

Knowing Paul as I do I can assure you that he would have had plenty more lucrative career opportunities that he could have pursued. Why did he choose Rotherham United and why has he stayed with us? It's quite simple really, it's because he's a life long Miller like the rest of us, and nobody will be hurting more than Paul Douglas about this situation. So unless you know all the facts, Stop trying to apportion the blame.

I believe the fault is in the system and not in the person. Human error is exactly that and is fundamental in making us what we are - Human Beings. There should have been and should be a "Dual Control" system in place for matters of this importance. Institutions like banks used to have these in place when I worked there 30 years ago. I suspect they don't now which is why so many errors are made within those banks and similar institutions. The difference however, is that they choose to take the risk of errors being made and take out sufficient PI (Professional Indemnity) cover to pay for errors which are then made and result in a financial loss. It's their calculation that the premium is cheaper than the cost of employing dual control procedures.
In the case of RUFC, no such Indemnity insurance can be taken out. How do you insure against an administration error leading to relegation? You can't, you might be able to insure the financial pain of the fine for an error being made in a late registration but not the relegation aspect itself.
So RUFC should exercise dual control on all elements of their procedures where an error that can be easily made by an individual, cannot be made by two people working independently of each attaching themselves to the same transaction. A simple but very effective process like this would eliminate errors of this magnitude which could have devastating repercussions on the club's immediate future. That is not to say I think at all that should we be relegated we will be in financial ruin, we won't of course given the strong accounts and the backing of TS. But, the effect on wages of players, contracts, ambitions of the current squad to be wanting to play in the Championship and others within the club may well lead to a wholesale change of personnel etc. given the change in status of the club (In League 1) with the commensurate loss of TV money, reduced crowds, reduced commercial and match day receipts etc. etc. etc.
So the lesson for me is not to sack anyone but to learn from the error and employ more robust systems.
Whether or not Paul Douglas is ultimately responsible for someone in his department making an error of any magnitude is meaningless. Of course he is responsible but it was the system in place that allowed the error to be made. Getting rid of Paul Douglas would be hideous. He is a vital ingredient in getting this club moving forward on an even keel. As far as I can guage, he has done a very good job so far and I hope he still has the full support of TS and others.
For me, you just need to change the procedure.

^^^^ I agree, learn from it and move on. Whoever made the mistake didnt di it on purpose, and probably will be hurting from the situation. We all make mistakes and if you have anything about you, you learn from them and become a better person.
The FDC will make a decision which they believe to be correct. We may or may not agree with the outcome. That hasn't stopped me from renewing my season ticket though.
